Holistic Alliance Care (HAC) puts people with special care needs at the centre of their
care. At HAC we enable people to determine how they are being cared for and we
are here to support your decisions from beginning to end. We understand that being in control is particularly important for people with complex care needs, and that is
why our services are tailored to individual preferences to achieve your specific health
goals whatever that would mean for you.
Some of the specialised services provided by our highly qualified and experienced
team including Registered and Clinical Nurses are:
- Diabetic care and management
- Enteral feeding e.g PEG feed, NGT feed
- Palliative/End of life care
- Management of severe dysphagia
Activities of daily living
Holistic Alliance Care provides flexible and customised support services for people
living with disabilities in a way that supports their health and well-being. We provide
one-on-one support on a day-to-day basis either in the home environment, community, work, school etc. Our specially trained and experienced support workers provide the following support services:
- House cleaning
- Gardening
- General household chores
- Meal preparation/Feeding assistance.
- Transport assistance to appointment or groceries shopping assistance
- Support one on one community engagement and participation




Mental health support services
At Holistic Alliance Care, we understand that an individual’s mental health and well-being are as important as their physical health. Like any other chronic condition, mental
illness can affect your ability to engage meaningfully in the community. At HAC, we
provide individually tailored support to vulnerable people who have an intellectual,
cognitive, sensory, physical, or neurological disability with co-morbid mental illness.
our role is to provide support and encouragement to develop an individual's skills,
support their independence, assist relationship development, maintain good health and well-being, and facilitate participation, contribution, and inclusion in the community. Other mental health support we offer include but are not limited to:
- Behavior support plan
- Positive behavioral activation
- Medication assistance and monitoring compliance
- Management of severe dysphagia
